Have you ever been made bankrupt?

Unfortunately, if you have previously been made bankrupt, we cannot proceed with your claim.

When you click submit below:

  • An email will be sent to the email addresses you provided which will contain a link for you to sign your claim documents
  • You will need to review these documents titled:
    • Letter of Authority
    • Terms and Conditions
  • If you are happy to proceed, you will need to provide your signature
    • Use your finger, stylus or mouse
    • Make sure it is a true likeness of your signature
    • If you are using a smaller device, please change to landscape orientation for ease of use
    • When you are satisfied with your signature, click on the button: Accept and Submit
